50 Minutes (face-to-face initial GP consultation) *Recommended for first appointments*

These appointments are for all new patients who are not registered with SDHC Private GP Service. All new patients are required to complete a new patient questionnaire prior to their appointment. This allows our GPs to review your information in advance.

Once your appointment is booked, our new patient questionnaire form will be emailed to you, however if you are unable to complete this online, please arrive 15 minutes before your appointment time to complete this at our reception desk.

These appointments are available for all registered patients who wish to be seen by our private GPs for a 50-minute GP appointment.

This fee covers phlebotomy service and is separate from the cost of any required blood tests. A GP consultation is needed beforehand to determine the necessary tests. Phlebotomy appointments cannot be booked without this initial consultation.
