Extended Access Service

SDHC successfully delivered 1,305 appointments to patients, ensuring timely access to care:

  • 466 face-to-face appointments
  • 839 telephone consultations

Without this service, patients indicated they would have sought alternative forms of care:

  • 42% would not have sought medical assistance
  • 31% would have called 111
  • 27% would have attended Urgent Treatment Centres or A&E
  • 1% would have contacted 999
 

Care Home Out-of-Hours (OOH) Service

The Care Home OOH service provided 184 consultations for care home residents, resulting in significant system-wide benefits:

  • 179 calls to 111 avoided
  • 18 calls to 999 avoided
  • 10 potential hospital admissions prevented 

Top reasons for consultations:

  • Complex frailty care (34%)
  • Medication management (19%)
  • Respiratory issues (17%)
  • Urology concerns (7%)
  • Palliative Care (7%)

Vaccination Campaigns: COVID-19 and Flu

Our vaccination Campaign efforts made a significant impact on community health:

COVID-19 Vaccinations 

  • Special Clinics: 2 Dedicated COVID-19 vaccination clinics for children with special needs (6m- 12y and 12-17) and individuals with hearing impairments
  • Care Home Patients Vaccinated: 379
  • Housebound Patients Vaccinated: 360
  • Outreach Events in Underserved Areas: 634 vaccinations administered
