Person-Centred Healthcare at SDHC
Healthcare should always be personalised, empathic, and built on a lasting relationship between doctor and patient. That's why, at SDHC, we believe in a person-centred approach. We don't just see ourselves as a healthcare organisation, but as a team dedicated to providing exceptional services focused on each person’s health and wellbeing.
But what exactly is person-centred healthcare? And why is it so important to the communities we serve here in East Sussex?
For those that may not be familiar with us, we are a GP federation established in 2016 to support the East Sussex community. A GP federation is a group of healthcare providers that come together to share resources, expertise, and best practices, ultimately improving the quality of care for all people.
There are currently around 200 GP federations in the UK, with us being one of them. We work collaboratively with 22 GP practices to deliver exceptional healthcare to the NHS and also via our private healthcare service. Currently, we serve over 300,000 people across Eastbourne, Hailsham, Seaford, Bexhill, Lewes, and High Weald.
Our team of highly experienced GPs, are all passionate about bringing quality care to the community. After all, everyone deserves a healthcare service that is tailored to their individual needs.
Our person-centred healthcare services
We provide extensive GP services for all common medical conditions, as well as referrals to specialists across East Sussex. We understand that GP appointment times can sometimes be inconvenient, particularly if you work full time. Therefore, we provide out of hours GP services in evenings and at weekends on behalf of local GP practices.
This service allows local GP practices to book their patients in with SDHC to be seen during weekday evenings and on Saturdays, too. We also provide COVID vaccinations and a care home service that offers remote weekend GP and bank holiday support to local care homes.
All our GPs are highly trained professionals with extensive experience working with people in East Sussex, so you know you are in safe and capable hands with us. They are familiar with local clinical pathways and can refer to local consultants and specialists should you need it. This ensures a coordinated and integrated approach to your overall health, ensuring you receive the best treatment possible.
What is person-centred care?
Person-centred care is the foundation upon which all healthcare should be built. Person-centred care is about focusing on the needs of each person and providing a personalised service that best serves and supports you. When we feel empowered to take charge of our health, we live happier, healthier lives, which in turn benefits everyone around us. We provide a wide range of NHS and Private GP services, all delivered with a focus on accessibility, efficiency, and, most importantly, you.
Health Checks
One of the Private GP services we offer is a Health Check, a service which allows people to gain a comprehensive and holistic understanding of their current health.
GP Health Checks consist of an initial 30-minute GP consultation, during which you can discuss their health concerns with one of our experienced GPs. Together, you then mutually decide on next steps, including which examinations and screening tests might be relevant for you.
A follow-up GP appointment is then arranged, during which you can discuss your test results with the GP, address any concerns or expectations, and decide on the next steps to optimise your health.
